Before you start the process you will want to make sure that you have gathered the following information:

 

·        Name of the church you attend

·        Pastor’s name & email

·        Name & email of a personal reference

·        Emergency contact name & number

·        Insurance Info

·        Medication Info (if applicable)

·        Immunization Info (just need to know what shots you have or have not had)

·        1st, 2nd & 3rd choice of activities you’d like to serve in. (See list)
