| Returning Volunteer Staff at SEP So Cal Thank you for deciding to apply to be Volunteer Staff again at SEP So Cal!!!Check to make sure you're available June 30th - July 7, 2012 - Staff Training: June 30, 2012 - Camp Dates: July 1-7, 2012
Applications for next summer will be available in 2012 Before you begin, please gather the following: - Name of the church you attend - Pastor's name & email - Name & email of a personal reference - Emergency contact name & number - Insurance Info - Medication Info (if applicable) - Immunization info (just need to know what shots you have or have not had) As you know, the application process has multiple steps: 1) Complete Online Application Form 2) Print, Sign & Return Consent Form 3) Send Pastoral Recommendation form to a Pastoral Reference 4) Send Personal Recommendation form to a Personal Reference 5) Get Livescan Fingerprinted unless you've been livescanned for SEP So Cal previously. You only need to do this once for SEP. If you live outside CA there is a different screening process. Call or email SEP for info.) 6) Mail us a copy of your insurance card (if applicable)
